Activities of Daily Living (ADLs)

ADL services include:
Eating & Feeding Assistance – Support with utensils, cutting food, and ensuring safe, comfortable mealtime experiences.
Toileting Assistance – Help with transferring, hygiene, clothing management, and use of catheters or ostomy care when required
Bathing & Personal Hygiene – Assistance with showering, sponge baths, hair washing, and drying.
Grooming – Hair care, oral hygiene, shaving, nail care, and maintaining a neat personal appearance.
Dressing – Support with clothing, fasteners, and adaptive devices such as braces or splints.
Transferring – Safe assistance moving from bed to chair, standing, or repositioning to prevent skin breakdown.
Mobility Support – Help walking, using assistive devices, or safely moving throughout the home.
Instrumental Activities of Daily Living (IADLs)
In addition to personal care, we also help with important household and wellness tasks that support independence:
Medication Assistance – Help with setup, reminders, and monitoring of prescribed and over-the-counter medications.
Meal Preparation & Cleanup – Planning menus, preparing nutritious meals, serving, and cleaning afterward.
Laundry & Light Housekeeping – Washing, folding, organizing clothes, and maintaining a clean home environment.
Shopping & Errands – Grocery, pharmacy, and essential supply shopping.
Complex Care Tasks – Specialized care approved by a medical professional, such as catheter or colostomy care, wound care, range of motion exercises, and skin integrity monitoring.

Our Process
At H & G Home Care Agency, we guide clients and families through every step of arranging quality home care — whether services are funded through Medicaid, state assistance programs, or private pay.
We make the process simple, transparent, and personalized, so you or your loved one can focus on what truly matters — living comfortably and confidently at home.
Step 1: Determine Eligibility
We start by helping you understand your care options.
Home care services can be funded in several ways:
Michigan Medicaid Home Help Program (MDHHS): For Medicaid-eligible individuals who need help with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
Veteran Assistance Programs: Certain VA benefits may cover home care services for qualifying veterans and spouses.
State and Community Support Grants: Programs such as the Michigan Aging & Adult Services Agency and local community partnerships may assist with care costs.
Private Pay: For families who prefer flexible, personalized scheduling and care options outside of Medicaid.
Step 2: Assessment & Care Plan
If applying through MDHHS, your caseworker will complete an in-home assessment to determine your approved care hours.
For private pay or other programs, we conduct our own free in-home or virtual consultation to evaluate needs, discuss preferences, and design a customized plan.
Step 3: Provider Selection & Setup
You can choose your own caregiver or allow H & G Home Care Agency to provide one of our trained, background-checked professionals.
If you choose your own caregiver, they must be enrolled and approved through the State of Michigan’s CHAMPS (Community Health Automated Medicaid Processing System).
We’ll assist with the CHAMPS registration process if they do not already have a Provider ID.
Our agency handles all necessary paperwork, scheduling, and coordination, ensuring a smooth transition into care and compliance with state requirements.
Step 4: Services Begin
Once authorized or scheduled, care begins at your convenience.
Our caregivers provide support with approved ADLs, IADLs, and other personal care needs — always delivered with empathy, respect, and professionalism.
Step 5: Ongoing Communication & Support
We stay connected with you, your family, and your caseworker to ensure your plan remains current and effective.
Our supervisors conduct regular quality checks and adjust services as your needs evolve.
